Description

SUMMARY 

Works with assigned participant(s) in an individual or group community-based setting to provide an environment for participant(s) to succeed in meeting the goals on their individual treatment plans.  The Community Support Workers act as role models and mentors in motivating the participant(s) to succeed. 

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES 

  • Create and maintain a positive environment that encourages success for participants through positive personal traits, motivation, and appropriate behavioral expectations. 
  • Be thoroughly familiar with the goals found in the participant’s individual treatment plan and provide support and services related to those specific goals. 
  • Provide safe transportation between the arranged pick-up locations and the community site. 
  • Enforce the policies and procedures of Pyxis. 
  • Maintain contact with participants, assigned program manager, participants families, guardians, and, as needed or assigned, the referring agencies. 
  • Report promptly to assigned community sites, participate alongside participants to guide participants in planned activities and work to develop a mentoring relationship with assigned participants.  
  • Complete all clinical documentation, attendance sheets, and other paperwork in an accurate and timely manner as directed by your program manager. 
  • Attend all required clinical supervision and other required training as assigned. 
  • Other duties as assigned.   

KNOWLEDGE, SKILLS, AND ABILITIES REQUIREMENTS

  • Strong interpersonal and communication skills. Ability to develop a professional, supportive, mentoring relationship with each participant that motivates them to succeed. 
  • Strong moral character, integrity, work ethic, and a desire to help others succeed. 
  • Attention to detail and ability to handle paperwork and clinical documentation in an accurate and timely manner. 
  • Reading, writing, and math skills as would be acquired through a high school diploma. 
  • Minimum of two (2) years of experience in a human services field or a four-year college degree in a human services field. 
  • Complete Behavioral Link’s policy and procedure training, on-the-job-training, CPR training, and First Aid training within 90 days of hiring. 
  • Have a clean driving record and maintain proper auto insurance. 
  • Community Support Workers that provide Respite Care must be 21 years of age, have a minimum of 2 years experience working with children with serious emotional disturbance or be equivalently qualified by education in the human services field and have Crisis Prevention Intervention (CPI) training within 90 days of hiring. 

ENVIRONMENT, COMMITMENT, AND PHYSICAL REQUIREMENTS 

  • Must be able to transport a group of 4 participants to and from a community site. 
  • Some job sites may require outside work in hot and/or cold weather. 
  • Must be able to participate alongside the participants in the assigned community setting. 
  • Must be able to continuously accompany the participants for up to 6 hours with no private breaks – only group breaks, and no smoking allowed. 
EMPLOYEE BENEFITS OFFERED
  • Mileage reimbursement for the usage of your personal vehicle will be $0.55 per mile driven while serving clients.
  • Phone usage reimbursement will be provided at $20 per month.
  • When you are considered FULL TIME, you will accrue 4 hours of PTO per paycheck. You will also accrue an additional half hour for every additional year you work at Pyxis.
  • FULL TIME staff will receive up to $100 towards their Insurance, and up to $200 towards insurance if they have been working for Pyxis for over a year.
